NAME OF MINOR, CONCENTRATION, OR TRACK FOR PROPOSED CHANGE:
Enter the full name of the degree or Minor, Concentration or Track program in which the change is being proposed.
NAME OF PARENT DEGREE PROGRAM OF MINOR, CONCENTRATION OR TRACK:
Within what parent degree program major (or major), does or will the minor, concentration or track lie? (i.e., Race & Ethnicity concentration is within the B.A. in American Studies degree program) This name should be consistent with what appears in the Undergraduate or Graduate Reocrd.
SPONSORING SCHOOL:
From the dropdown menu provided, select the school(s) requesting the proposed change.
PROPOSAL CONTACT FULL NAME:
Indicate the name of the primary contact at the sponsoring school with whom IRA staff will work to manage the steps required to approve the proposed Minor, Concentration or Track change. If there are additional contacts closely involved in this proposal who you would like IRA staff to include on related correspondence, please provide their names and email addresses in the Description of Changes to Existing Degree or Minor, Track or Concentration program attachment (see below).

For addition or modifications of a minor, concentration or track specify the total number of credits for the collection of courses being requestd. For closure of a minor, concentration or track indicate "not applicable."
DESCRIPTION OF CHANGES TO EXISTING MINOR, CONCENTRATION, OR TRACK:
Upload a Word document or PDF or describe directly on this form the nature and rationale behind the proposed change addition, modification or closure of the Minor, Concentration or Track program specifying the last time that a change to the parent degree program of a similar nature was requested.
Identify other units that are active in this space/discipline across grounds and detail the consultation undertaken with those units to assess and manage any anticipated impacts. For a concentration, minor or track closure, in addition to describing the reason for the request, please specify proposed effective end date and number of students currently enrolled in program.
PERCENT OF NEW COURSES:
This question pertains only to the core courses that students in the proposed Minor, Concentration or Track will be required to take. For example, for a 15 credit-hour Minor, comprised of five required courses, the percent of the courses that are new. For a closure, please indicate not applicable.
FACULTY RESOURCES:
Indicate whether the school plans to hire new faculty to teach the core courses required for the proposed Minor, Concentration or Track. In addition, indicate the location(s) of existing faculty that you anticipate will teach the core courses of the proposed Minor, Track or Concentration. For example, will all existing faculty teaching the core courses come from the sponsoring school? For a closure, please indicate not applicable.
REQUIRES NEW RESOURCES:
Indicate whether the new Minor, Concentration or Track will require new library acquisitions/ subscriptions; and/or any other new learning resources, such as instructional/research software. (select all that apply)

Indicate whether the new Minor, Concentration or Track will require new equipment or physical space beyond the school’s current resources.
DELIVERY METHOD:
Indicate the delivery format(s) in which students can complete the parent degree program under which the minor, concentration or track is housed. (select all that apply):
- If a student is able to complete the entire degree program (i.e. the total credit hours required for the degree) in a traditional, Face to Face format with no online coursework, select ‘100% Face to Face.’
- If a student is able to complete the entire program online, with no face-to-face coursework, select ‘100% Online.’
- If a student is required or able to complete the program via a combination of Face to Face and Online courses, indicate ‘Hybrid.’
PROGRAM LOCATION:
Indicate where the parent program under which the minor, concentration or track is housed is held (select all that apply):
- If the program is offered at the main campus in Charlottesville, select that entry.
- If the program is offered at an approved UVA off-grounds location, select ‘Approved UVA Off Campus site.’
- If the program is offered 100% online, i.e. students are able to complete the entire program remotely, select ‘100% online.’
INTERNSHIP OR PRACTICUM REQUIREMENT:
Indicate whether the Minor, Concentration or Track does or will require students to complete an internship, practicum, or clinical experience.
LEADS TO PROFESSIONAL CERTIFICATION OR LICENSURE:
Indicate whether the changes to the parent degree program resulting from the addition or modification of the minor, concentration or track will formally or potentially lead to professional licensure or certification (i.e. the addition or modification of courses for the minor, concentration or track may create a path for a professional license/certification in Virginia or other US states).
DATE OF SCHOOL CURRICULUM COMMITTEE APPROVAL:
Indicate the date on which the proposed minor, concentration or track change was or will be reviewed and approved by the internal curriculum committee of the proposing school. If the committee meeting schedule is not yet known at the time of submission, indicate 09/01/YY if you anticipate review in the coming Fall semester, and 02/01/YY if you anticipate review in the coming Spring.
DATE OF DEAN APPROVAL:
Indicate the actual or anticipated date on which the proposed degree or Minor, Concentration or Track program change was or will be reviewed and approved by the dean of the proposing school.
