ARTICULATION (TRANSFER) AGREEMENT:
A pathway that clarifies how courses taken at another institution apply toward completion of a baccalaureate degree at UVA. These can be formed between a system and individual UVA schools (e.g., VCCS and School of Engineering; VCCS and College of Arts and Sciences) or between a specific school (e.g., Richard Bland College) and a UVA school (e.g., College of Arts and Sciences).
A location geographically apart and independent of UVA’s main campus in Charlottesville that is: 1) permanent in nature; 2) offers courses in educational programs leading to a degree, certificate, or other recognized educational credential; 3) has its own faculty and administrative or supervisory organization and; 4) has its own budgetary and hiring authority.
CENTER OR INSTITUTE:
An organization other than an academic department or administrative unit within a school or a unit reporting to the Office of the Executive Vice President and Provost (EVPP) created to enhance the educational, research, and/or service missions of the University. Organizations created outside a school or unit reporting to the EVPP are also considered an academic center or institute if their activities will involve academic faculty members and their mission is to enhance the educational, research, and/or service missions of the University.
A proposed new program offered by two or more UVA schools wherein each assume an equal role in the academic oversight of the program, or an agreement between two schools designed to expedite completion of two existing degree programs through some reciprocity of credits.
An agreement with another university designed to facilitate completion of two existing degree programs through some reciprocity of credits. The agreement results in separate program completion credentials each of which bears only the name, seal, and signature of the institution awarding the degree to the student.
A single program completion credential bearing the names, seals, and signatures of each of the two or more institutions awarding the degree to the student.
Any type of action not covered above or through another initiation form request.

PROPOSAL CONTACT FULL NAME:
Indicate the name of the primary contact at the sponsoring school with whom IRA staff will work to manage the steps required to approve the change. If there are additional contacts closely involved in this proposal who you would like IRA staff to include on related correspondence, please provide their names and email addresses in the file attachment (see below).

REQUESTED EFFECTIVE TERM OF PROPOSED PROGRAM/ACTION:
Enter the term in which the school wants the proposed action to be effective.
Upload a brief description (one page or less) word document or PDF describing the purpose of the request. Please specify the reason for the change, what you aim to accomplish through the proposed action, and if you are collaborating with another department within or outside of UVA.
If related to a dual degree, please specify the UVA degree program, and the name and degree program of the partner University. If related to a joint degree, please specify the proposed degree program and academic departments at each school who would oversee the program.
If you selected “Other” as the type of action you are requesting, please provide necessary context for IRA staff to initially process.
DATE OF SCHOOL CURRICULUM COMMITTEE APPROVAL:
Indicate the date on which the proposed change was or will be reviewed and approved by the internal curriculum committee of the proposing school. If the committee meeting schedule is not yet known at the time of submission, indicate 09/01/YY if you anticipate review in the coming Fall semester, and 02/01/YY if you anticipate review in the coming Spring.
DATE OF DEAN APPROVAL:
Indicate the actual or anticipated date on which the proposed change was or will be reviewed and approved by the dean of the proposing school.
CONTRACTUAL AGREEMENT OR CONSORTIUM:
Indicate whether the new degree or any of its constituent courses will be delivered through a contractual agreement with another University or other party.
